I, the undersigned parent or legal guardian, understand that participation in Mondial Athletics LLC’s Summer Soccer Workouts and related programs involves physical activity and carries a risk of injury. I hereby release and hold harmless Mondial Athletics LLC, its coaches, assistants, staff, and the facility (Tanja King Park) from any and all liability, claims, demands, or causes of action for injuries, damages, or losses that may occur during or in connection with participation in this program.
I certify that my child is physically fit and capable of participating in the activities and that I am responsible for providing all necessary equipment, including appropriate footwear (cleats) and a soccer ball.
By acknowledging below, I confirm that I have read, understood, and agreed to the terms of this Waiver & Release of Liability.
